Individual doctors have very little agency anymore. Most of what they can and can't do is heavily controlled by state medical boards, hospital administrators, and the joint comission.

The joint comission decides if your hospital gets paid for Medicare patients or not so every hospital will do everything they can to not piss them off. Back at the beginning of the opioid crisis they stated that doctors weren't treating pain well enough. Essentially hospitals had to give people pain meds in order to get paid. Every single person on the joint comission at that time was a paid consultant for at least one pharma company.

Your family doctor doesn't get to decide shit. If they don't do what their told they will lose their job or possibly their medical license. The whole system is shit.
