I was visiting someone in a hospital one day and an old lady in another bed was being grilled by a doctor and a nurse. She was in her late 80's, and her daughter was with her. They found cancer that had spread, so the staff acted like a police interrogation unit for half an hour to try to get her to say she had smoked at some point in her life. She was from some rural back parts of China, and had never had a cigarette and nobody in her family smoked, but the staff didn't want to hear that.
That was in a socialized medicine country. They can't prove anything about cigarettes or what caused your cancer. Not ever. Not once, but the belief and fear is so deep it has to win no matter what.
