Doctors are very risk-averse. Most of the time, they're looking for some authority to tell them what the consensus is so that they can just implement/repeat that and fall back on the authority if someone questions them or something goes wrong. The consensus, in turn, is determined by Big Pharma -- often literally written by them. The safe thing to do is to prescribe whatever pill or use whatever device they're selling.
