Looking straight at its' face, the left eye is photoshopped inwards to make her cockeyed. The cheeks are exaggerated a bit. All in all, pretty subtle and just for extra laughs, she's already hideous.

Looking at the un-shopped version I'm not sure which is actually more terrifying. The unshopped version has "crazy eyes", versus the cartoon-like "cross-eyes" of the shop (which make her look funny and less harmful).
