Getting stabbed by something rusty doesn't guarantee tetanus. The bacteria has to be present, and your body has to not be able to defend itself against it for whatever reason.
It's like E. coli. It is ever present in our digestive system, but if there is too much of it in a food because Paco didn't wash his hands after wiping his ass, you MAY get sick depending on your immune system and your overall health. If your body is used to handling X amount of bacteria and suddenly there is 100X of that bacteria present, it may not respond quickly enough to prevent symptoms.
If you don't believe this shit is real, please go eat Indian food from a street vendor and find out for yourself.
All you said just indicates that I live a clean, balanced life. Lots of things are real, but that doesn't mean it is real for everybody. For now, I don't live in the hellscape where people just drop trou and take a dump anywhere. This is what we fight here, however effectively. i got my feet punctured by a nail a few years ago. somebody left a nail in my community park and that crap pierced me thru my sandals. i had to go the ER because of it, got the wound disinfected there and got a tetanus shot.

Ive stood on and stabbed myself with many rusty objects, the only time I ever really considered a tetanus shot was cutting my hand deep on 15 year old chinese laminate but ultimately decided against it.
I've had tetanus, it's definitely real. Obviously, getting it isn't a death sentence but it's still no fun. Treatment is antibiotics and a tetanus shot. Symptoms include a high fever and your hands locking up. This was like 15 years ago and I still have the scar to remind me.
You can also get lockjaw. I know someone back in the 80's who stepped on a rusty nail and ran it through their foot and they didn't have the tet shot and they were sick for a little while.
A vaccine us a weakened form of the disease a pushover to train the immune system to spot certain diseases and how to defeat them by arming them with antigens. A vacination durring a a sickness can bebeffective if the immune system isnt beating the pathogen on its own, since he was receiving an anti biotic he likely would not have had his immune system in full gear om the job.
Likely they gave him the vaccine in the hospital, then gave him a prescription for an antibiotic and told him to wait 12-24 hours after receiving the shot to take it.
The vaccine might have been superfluous but we aren't conducting research that would indicate that and tetanus kills so it's a better safe than sorry situation, a tetanus shot is like maybe 20 bucks anyway, better than lockjaw and death.

The last time I went to a medical facility for anything was because I got celulitis in a city while working a traveling labor gig. That shit hurt so bad I practically crawled into the urgent care. City people fear these things because they live around filth, so random shit can in fact kill you.
I'd settle for it not being enough of a threat that deserves shots for normal people. I'm sure there are all sorts of weird bacteria out there that you can get if you're really unlucky.
Tetanus vaccine is just a bunch of the toxin, so your body can have a resistance to it, and not get killed by the toxin when it's coming in greater number from the bacteria
It is real but if you get cut by something rusty, let it free bleed. There was a little boy in Oregon that got it on his sheep farm and he went to the hospital only because they sewed up the cut on the farm instead of letting it bleed a little first.
