why is out of pocket insurance os expensive in the US?

Budesonide has been around for like what, 50+ years? And yet it's still hundreds of dollars in the US. Budesonide with insurance is 500$. Budesonide in any other country is like 25$.

I don't know why medication is expensive, but "affordable care act" told insurance companies Daddy government will pay the first few hundred dollars of a premium so all premiums start at that amount.
