It is repulsive to me. I see it, and I wonder "Where is this man's pride? How do you let yourself look like that and feel okay with it?" I feel secondhand embarrassment for these people.

And I say this as a man who grew up fat. I had to turn it around myself in my early teens- poor diet was the biggest issue for me, but I decided I didn't want to be fat anymore and worked hard. At age 13-14, you can really change your body quickly. I wonder if all that work I put in back then contributed to me remaining slim all these years later. I don't drink alcohol, ever, so I'm certain that helps.
